The below forms can all be downloaded and completed in Adobe Acrobat or alternatively, printed and filled out by hand.

• Please complete the "Intake" form to outline your referral information. This is a general form and is not required prior to seeing Dr Daff.
• Please complete the "Consent" form when requested by Dr Daff, to allow her to communicate with others involved in your care (for example, your GP if you are attending with a Mental Health Care Plan).
• It can be helpful to have thought a bit about what you might like to be different after seeing a psychologist. The "Goals" sheet below provides some prompts for thinking about this. This is a general sheet and can be discussed and tailored with your clinician.
• The "DASS-21" is a freely available validated screening measure for mood related concerns. Please complete the DASS-21 if requested by Dr Daff during the course of your treatment.
• The "APS Charter" is produced by the Australian Psychological Society and summarises your rights as a client.
• The "Privacy Policy" summarises how personal information will be collected, stored, and used and is for client information.
